Permission to Withhold Student Directory Information
This form explains how directory information is used and allows parents to have their student's information be withheld. Families who want to withhold information must submit a new form every year.
Parents whose income falls within specified guidelines set by the U.S. Department of Agriculture should apply for free or reduced-price meals for their students.
