If you need to take medically necessary leave lasting more than 10 working days, review this section for guidance.

1. Read "Applying for a Medical Leave Handbook". 
2. Read the Employee Rights Under FMLA.
3. Read the Employee Rights Under PFML.
4. Submit the Leave of Absence Application: Complete the Leave of Absence Application form electronically.
5. Download the Employee Health Care Provider Certification and request your medical provider to complete the document.
6. Submit the Employee Health Care Provider Certification to your HRBP.
7. Submit Medical Authorization to Return to Work Form two weeks prior to the end of your leave.

OPTIONAL: Leave Sharing Application (Completed by employee to request donated leave)

If you need to take more than 10 working days of leave to care for a family member due to a medical event, please review this section.

1. Read "Applying for Leave to Care for a Family Member Handbook". 
2. Read the Employee Rights Under FMLA.
3. Read the Employee Rights Under PFML.
4. Submit the Leave of Absence Application: Complete the Leave of Absence Application form electronically.
5. Download the Family Member’s Health Care Provider Certification and request the medical provider to complete the document.
6. Submit the Family Member’s Health Care Provider Certification to your HRBP.

OPTIONAL: Leave Sharing Application (Completed by employee to request donated leave)

If you are an employee who requires leave after the adoption or birth of a child, please review this section.

1. Read "Applying for Parental Leave Handbook". 
2. Read the Employee Rights Under FMLA.
3. Read the Employee Rights Under PFML.
4. Submit the Leave of Absence Application: Complete the Leave of Absence Application form electronically.

If you are the birthing parent, and you are taking a medical leave prior to the baby’s due date, you are required to submit an Employee Health Care Provider Certification.

5. Download the Employee Health Care Provider Certification and request your medical provider to complete the document.

6. Submit the Employee Health Care Provider Certification to your HRBP.
