If you need to take more than 10 working days of leave to care for a family member due to a medical event, please review this section.
If you are an employee who requires leave after the adoption or birth of a child, please review this section.
If you are the birthing parent, and you are taking a medical leave prior to the baby’s due date, you are required to submit an Employee Health Care Provider Certification.
5. Download the Employee Health Care Provider Certification and request your medical provider to complete the document.
6. Submit the Employee Health Care Provider Certification to your HRBP.
To apply for an Emergency Leave, you must meet one of the following criteria:
Leave necessitated due to a Call to Active Duty for employee or employee's spouse, state registered domestic partner, son, daughter, or parent.
A leave of absence without pay shall be granted under the following provisions:
