Athletics: Participation Forms
Prospective athletes and/or their guardians must have a valid physical on file. A physical is valid for two years, and must be valid through the entire athletic season including the state tournaments. You may either submit it online by scanning your copy or delivering the hard copy to the athletic office at your school.
FamilyID Sports Registration
We are excited to announce online registration through FamilyID (www.familyid.com). FamilyID is a secure registration platform. When you register through FamilyID, the system keeps track of your information in your FamilyID profile. You enter information only once for each family member for multiple uses and multiple sports.INFORMATION NEEDED TO REGISTER: It will be necessary to have the following information to allow for accurate completion of your online registration:
- Doctors information
- Health insurance information
- Student ID number
- Graduation year of your student.
Please remember when you provide emergency contact information for your athlete you need to give us two individuals in addition to yourselves as parent/guardians! YOURS will be the first numbers called, but in case you cannot be reached, we need two OTHER individuals you want us to contact.REGISTRATION PROCESS: A parent/guardian registers by following these steps: 1. Go to www.familyid.com. 2. Once on the Family ID page, select the NAME (under the word Programs on the blue bar). 3. If this is your first time using FamilyID, click Create Account. Once you have a FamilyID account, you can select Log In.
4. Create your secure FamilyID account by entering the account owner First and Last names (parent/guardian), E-mail address and password. Select I Agree to the FamilyID Terms of Service. Click Create Account. Save your username and password used to create your account.
4. You will receive an email with a link to activate your new account. (If you don’t see the email, check your E-mail filters (spam, junk, etc.). Click on the link in your activation E-mail, which will log you in to FamilyID.com5. Once in the registration form, complete the information requested. All fields with a red* are required to have a response.ALSO: Critical that you are careful when 'signing' the policy/agreements. Some REQUIRE PARENT; some REQUIRE STUDENT! Double check that you have done those correctly BEFORE submitting your registration.If you are unable to complete the form you can save the form and login later to complete the information. 6. Click the Save & Continue button when your form is complete. 7. Review your registration summary. 8. Click the green Submit button. After selecting ‘Submit’, the registration will be complete.
