LEOTA JUNIOR HIGH
ATHLETIC ELIGIBILITY PACKET
(Updated September 2009)
According to WIAA, District and Leota Junior High School policy, each athlete must complete the following prior to turning out for a sport.
All forms need to be turned in by 10:00 am the first day of tryouts (September 2nd). Please turn in ALL your paperwork together to the Main Office.
DO NOT TURN IN TO YOUR COACH
after practice has begun. You will be cleared for practice only after all paperwork has been reviewed.
1) ONCE EACH YEAR:
Complete and sign all forms in Eligibility Packet (can be obtained in the Leota Office)
a. SPORT PHYSICAL form (blue) completed in compliance with WIAA regulations (signed and dated by doctor). These physicals are good for 2 years.
b. EMERGENCY INFORMATION (green card)
c. ATHLETIC ELIGIBILITY (residency and grade eligibility) (gray)
d. STUDENT ATHLETIC CODE (yellow)
e. ASB, ATTENDANCE AND TRANSPORTATION FORM (white)
f. HIPA NOTICE OF PRIVACY PRACTICES (peach)
2. EACH SEASON :
SAFETY GUIDELINES (sport specific) - This form is not included in the Eligibility Packet. EACH SEASON you must complete the Safety Guidelines form for the sport you wish to participate in (i.e., Basketball Safety Guidelines, Baseball, Soccer, etc.). Form is available on the counter in the Main Office or on the District Website.
3. FEES
ASB CARD needs to be purchased before your first contest- $15.00
**PARTICIPATION FEE - prior to the first contest of each sport season
$100.00 fee for each sport. Individuals participating in more than one season will pay a maximum of $200.00 per school year.
Families
with two or more students participating at the same junior high school during the same season will be charged $70.00 per participant up to a family limit of $280 per year.
