Vision and Hearing Aid Hardware Plan
The Northshore School District's Vision and Hearing Aid Hardware Plan is self-insured by the District to assist in the cost of vision hardware (frames, lenses, contact lenses) or hearing aids. This is a mandatory benefit.

Vision Frames and Lenses
If an employee does not complete an enrollment form within 30 days of eligibility, they will be enrolled but claims will not be processed until an enrollment form is completed. Claims are processed by Zenith Administrators, Inc. The benefit provides for frames once every two calendar years, up to $80; lenses once every calendar year for single vision at $64; bifocal at $97; trifocal at $141; and lenticular at $155. Contact lenses in lieu of frames/lenses, are provided once every calendar year, up to $125. Certain refractive corrective surgery is provided up to a lifetime maximum of $500 per person.Eye exams are covered under your medical insurance coverage.

If you are legally blind, deaf-blind, visually impaired (cannot easily read conventional size print), physcially disabled (cannot comfortably hold books or turn pages), or learning disabled due to organic dysfunction and are a resident of Washington State, you can apply for Books on Tape. Hearing Devices
Hardware for hearing devices are covered under the District's self-insured plan. Claims are processed by Zenith Administrators, Inc. This coverage is a mandatory benefit, but claims will not be processed until an enrollment form is completed. Coverage provides for hearing devices once each 24 months up to $500. Prices can vary considerably for similar equipment from provider to provider so it is advised that participants compare costs prior to purchase. Hearing exams are covered under your medical insurance coverage. An audiologic examination and hearing evaluation may be included in conjunction with the purchase of a hearing aid.
